The Practice welcomes new registrationsGeneral Information The Practice area covers, but is not limited to: Worksop, Carlton, Langold, Firbeck, Stone, Woodsetts, Gildingwells, Oldcotes, Blyth, Styrrup, Tickhill, Harworth, Bawtry, Scrooby, Ranskill, Ranby, Hardwick (Clumber), Carburton, Cuckney, Osberton, Shireoaks, Rhodesia & Thorpe Salvin and surrounding areas. How to Register Please contact the surgery at which you wish to be seen. Inform the receptionist that you wish to register with the Practice. The receptionist will arrange an appointment time for a new patient registration check. This is a standard check that gives us a chance to meet you and to get some basic information (height, weight, family medical history etc) onto our clinical computer system. Not only does this help us to understand your medical needs, but it also allows us to plan what services are most suitable for all our patients. You will also be asked to bring a urine sample along to this appointment along with any medications you may take. New patients will be asked to complete registration forms prior to their appointment, these can be collected from the practice or downloaded from the ’Patient Forms’ tab on the left-hand side of this page. Your details will then be input on to the computer ready for your new patient appointment. Your notes from your old practice should be received into the practice within 6 weeks of application but this is quite often much quicker.
