To Register with the practice:

The practice requires a completed GMS 1 form and a new patient registration form in advance of your appointment to enable us to enter your demographic details onto the clinical system. Both forms can be accessed below.

You must make an appointment to have a new patient check, you are also required to bring along an urine sample to this appointment, if you do not bring one you may have to make another appointment.

Please note you are not registered with the practice until you have completed this process.

Welcome to Larwood & Village Surgeries. You are able to have an appointment to see a Doctor or Nurse at any one of our four sites, although we do recommend that you see the same doctor for ongoing problems. We also offer extended opening hours on Saturday mornings and certain evenings for patients that find it difficult to attend during our normal opening times. You can order your repeat prescriptions on this site by selecting one of the tabs down the lefthand side - Repeat Prescriptions or SystmOnline, note you will need a password generated by the surgery to use SystmOnline, which will take you though to a screen to choose your required current repeat medication.
